Incident Reporting & Liability Claim
Use the Incident Reporting & Liability Claim form to report all incidents that resulted in an injury or property damage or had the potential to cause an injury on University property or at a University activity or event.
Workplace Injury (Workers' Compensation)
Employees must report work-related injuries to their supervisor or other person designated by their department as soon as possible.
